Physicians show better adherence to Korean COPD guidelines than international GOLD recommendations. Treatment strategies are evolving to incorporate both Korean and recent GOLD guidelines, especially for patients enrolled after 2019.
Area of Science:
- Pulmonary Medicine
- Pharmacotherapy
- Clinical Practice Guidelines
Background:
- Chronic Obstructive Pulmonary Disease (COPD) management requires adherence to established clinical guidelines.
- Evaluating guideline adherence is crucial for optimizing patient outcomes in COPD care.
Purpose of the Study:
- To assess the adherence of respiratory specialists to Global Initiative for Chronic Obstructive Lung Disease (GOLD) and Korean guidelines for stable COPD management.
- To analyze trends in prescription patterns and guideline compliance over time.
Main Methods:
- Utilized medication data from the Korea COPD Subtype Study (KOCOSS) cohort (2011-2022).
- Categorized patients into pre- and post-2019 enrollment groups.
- Analyzed prescription patterns and calculated adherence percentages against Korean 2018, GOLD 2019, and GOLD 2023 guidelines.
Main Results:
- 85.6% of 3,477 patients received pharmacological therapy, with 81.6% using inhaled medications.
- Inhaler prescription rates increased post-2019 (79.7% vs. 86.7%), predominantly with dual bronchodilators.
- Adherence to Korean 2018 guideline was 56.9%, while GOLD 2019 and 2023 guideline adherence was 31.3% and 28.0%, respectively.
Conclusions:
- Korean guidelines demonstrated higher adherence compared to GOLD recommendations.
- Increased alignment with both Korean 2018 and GOLD 2023 guidelines was observed in patients enrolled after 2019.
- Physicians are adapting treatment strategies to align with evolving domestic and international COPD guidelines.
