Post-transplant lymphoproliferative disorder (PTLD) is a potential life-threatening complication after solid organ transplantation. PTLD can be associated with Epstein-Barr virus through either primary infection or reactivation during treatment with immunosuppressive medications. We present a rare case of Epstein-Barr virus-positive classical Hodgkin lymphoma PTLD following liver transplantation, which was initially misattributed to late transplant rejection and raised concerns for cholangiocarcinoma due to abnormal laboratory and imaging findings.
