Area of Science:

  • Orthopedic Surgery
  • Traumatology
  • Biomechanics

Background:

  • Optimal surgical approaches for complex tibial plateau fractures lack consensus.
  • Achieving anatomic reduction requires clear joint access.
  • Existing techniques like osteotomies offer improved visualization but have limitations.

Purpose of the Study:

  • To describe a novel surgical technique for complex lateral tibial plateau fractures.
  • To enhance visualization of the articular surface for accurate reduction.
  • To address limitations of current surgical approaches.

Main Methods:

  • A technical note detailing a specific surgical approach.
  • Release of posterior meniscocapsular fibers up to the popliteus tendon.
  • Folding the lateral meniscus into the intercondylar notch for exposure.

Main Results:

  • The described technique provides clear access to the articular surface.
  • Facilitates anatomic reduction of complex lateral tibial plateau fractures.
  • Avoids anterior horn detachment, potentially improving healing and reducing subluxation.

Conclusions:

  • This technique offers an effective method for visualizing the articular surface in complex lateral tibial plateau fractures.
  • It may lead to improved surgical outcomes and reduced complications.
  • Further studies are warranted to validate its efficacy compared to existing methods.
