    Background:

    • Bile duct injury (BDI) is a rare complication of blunt abdominal trauma.
    • Delayed diagnosis and management are common due to the infrequency of this injury.

    Observation:

    • A review of 94 cases and an illustrative case highlight diagnostic challenges.
    • Early signs like hypovolemia and acute abdomen often mask the biliary injury.
    • Late signs include jaundice and abdominal distention, indicating biliary compromise.

    Findings:

    • Delayed operation (>24 hours) occurred in 53% of patients.
    • Early surgery (within 24 hours) correlated with a higher incidence of injuries distal to the cystic duct and complete ductal severance.
    • Injuries distal to the cystic duct and complete severance showed a highly significant association.

    Implications:

    • Diagnostic paracentesis can aid early diagnosis.
    • Surgical exploration and cholangiography are crucial for management.
    • Individualized surgical repair based on injury characteristics is essential for optimal patient outcomes.
