Background:

Rotator cuff diseases (RCDs) are prevalent, affecting 5-39% of the general population and over 30% of individuals older than 60.
The complex anatomy and function of the rotator cuff make it susceptible to significant morbidity.

Purpose of the Study:

To systematically review the efficacy of collagen injections for treating rotator cuff diseases (RCDs).

Main Methods:

A systematic literature search was conducted across major scientific databases (PubMed, Scopus, Web of Science) up to November 2024.
Study selection and quality assessment were performed by two independent reviewers using PRISMA guidelines and the Modified Coleman Methodology Score (CMS).

Main Results:

Eight studies were included, generally demonstrating fair methodological quality.
All included studies reported clinically relevant improvements in pain and function following collagen injections for RCDs.
Significant heterogeneity among studies precluded definitive conclusions on optimal collagen types or injection protocols.

Conclusions:

Collagen administration appears effective for reducing pain, enhancing function, and improving tendon structure in RCDs, particularly for partial tears and tendinopathy.
High-quality, prospective studies with long-term follow-up are required to validate these findings and establish best practices.
