Purpose of the Study:

  • To evaluate the efficacy of specific ventilator setting adjustments in reducing pulmonary artery pressure.
  • To assess the impact of positive end-expiratory pressure (PEEP), hyperventilation (hypocapnia), fraction of inspired oxygen (FiO2), and alkalosis on pulmonary hemodynamics in a PE model.

Main Methods:

  • A randomized, blinded, cross-over study was conducted on eleven pigs following acute PE induction.

Conclusions:

  • In an acute PE porcine model, reducing PEEP, implementing permissive hypocapnia via hyperventilation, inducing alkalosis, and increasing FiO2 effectively lower mean pulmonary artery pressure.
  • These ventilator strategies show promise for managing pulmonary hypertension secondary to PE.
