Guidelines for Writing Outcome01:11

Guidelines for Writing Outcome

2.6K
When developing expected outcomes for a patient care plan, the nurse should adhere to the following recommendations:
Patient outcomes reflect the patient's response to the goal rather than what the nurse aims to achieve. Terminology should be observable and measurable to avoid the reader's interpretation. The desired outcome should be realistic and achievable in the designated care timeframe. Expected outcomes should align with adjunctive therapies. The outcome should enhance care...

Nursing Process for Patient and Caregiver Teaching III: Evaluation and Documentation

1.6K
Evaluation of the teaching process enables the nurse to determine if the patient's learning needs were met and if training was effective. If the expected outcomes are not met, the care plan is revised, and additional education or reinforcement is provided. Nurses can ask questions after the session or obtain feedback to assess the patient's understanding of the topic.
Nurses can use several methods to evaluate patient outcomes. For example, oral questions can assess cognitive learning,...

Nursing Evaluation01:15

Nursing Evaluation

3.2K
The evaluation stage signals the end of the nursing process. The nurse gathers evaluative data to assess whether or not the patient has attained the expected results. Whereas the nurse collects data in the nursing assessment to identify the patient's health concerns, the evaluation stage data determines if the indicated health issues are resolved. Evaluative data collection includes two sections: the data acquired to evaluate patient outcomes and the time criteria for data collection.

Community Based Intervention01:30

Community Based Intervention

27
Community-based interventions in mental health represent a paradigm shift from institution-centered care to treatments embedded within the fabric of local communities. By prioritizing inclusion and leveraging existing societal structures, this approach fosters a supportive environment conducive to addressing mental health challenges while promoting individual dignity and agency.

Methods of Documentation VI: Case Management Model

557
The case management model is a multidisciplinary approach that involves healthcare professionals from diverse disciplines, such as physicians, nurses, therapists, social workers, and pharmacists, working collaboratively to address the various needs of patients. Each healthcare professional brings unique expertise and perspectives, contributing to a more comprehensive understanding of the patient's condition and tailoring treatment plans accordingly.
For example, a patient with a chronic...

Role of Communication in the Nursing Process III: Evaluation and Documentation

1.2K
A successful patient outcome depends mainly on the evaluation stage of the nursing process. Evaluation determines effectiveness by reviewing what was done previously after the completion of nursing interventions. Every time a healthcare professional steps in or administers treatment, they must reassess or evaluate the action to ensure the intended result. During the evaluation phase, there are three probable patient outcomes:

Area of Science:

  • Healthcare quality improvement
  • Nursing education and practice
  • Interprofessional collaboration

Background:

  • The Evidence-Based Practice Mentoring Program (EBPMP) is a 12-month initiative at a Hawai'i acute care facility.
  • It features a two-tiered mentoring structure: EBP faculty mentor EBP mentors, who then guide mentees.

Purpose of the Study:

  • To evaluate the impact of a pilot EBP mentoring program on promoting the Value, Implementation, and Knowledge of Evidence-Based Practice (EBP).

Main Methods:

  • A literature search, appraisal, and synthesis informed the EBPMP development.
  • The pilot involved a nurse-led interdisciplinary team from July 2022 to June 2023.
  • Outcomes measured included the Quick-EBP-VIK survey, self-confidence in EBP mentoring skills, feasibility, and program cost.

Main Results:

  • Significant increases were observed in EBP Value (11.14%), Implementation (80.87%), and Knowledge (99.46%) for mentors and mentees.
  • Mentors' self-confidence in EBP mentoring skills substantially increased over 12 months.
  • Program feasibility was high, with 80% of mentees completing modules and an 88.1% meeting attendance rate.

Conclusions:

  • EBP mentoring is an effective strategy for enhancing staff EBP development and fostering an organizational culture of EBP.
  • Sustained leadership support is vital for the EBPMP's long-term success and sustainability.
