Summary

Robotic surgery for complicated diverticulitis showed similar outcomes to laparoscopy, with no significant differences in reoperation or complication rates. However, robotic colectomy led to increased operative time but reduced blood loss.

Purpose of the Study:

  • To compare outcomes of laparoscopic versus robotic elective sigmoid colon resection for complicated diverticulitis.

Main Methods:

  • Retrospective review of 131 patients undergoing elective minimally invasive sigmoid colectomy for complicated diverticulitis (2018-2023).
  • Analysis included reoperation, complications, conversion rates, blood loss, operative time, return of bowel function, and length of stay.

Main Results:

  • No significant differences in reoperation (7.7% vs 2.1%), complications (5.1% vs 8.4%), or length of stay (5.2 vs 5.2 days).
  • Robotic colectomy resulted in significantly longer operative time (128.11 min increase) but less estimated blood loss (33.4 cc decrease).

Conclusions:

  • Robotic sigmoid colectomy for complicated diverticulitis yielded comparable outcomes to laparoscopic approaches.
  • While robotic surgery reduced blood loss, it was associated with an extended operative time.
