Purpose of the Study:

  • To investigate the influence of ONH rotation and tilt on S-F relationship differences in early OAG with and without HM.
  • To analyze how ONH morphology affects the correlation between peripapillary retinal nerve fiber layer thickness (pRNFLT) and visual field sensitivity loss (VFSL).

Main Methods:

  • Included 164 eyes: 69 early highly myopic glaucoma (HMG), 60 early OAG, and 35 healthy HM.
  • Utilized spectral-domain optical coherence tomography, standard automated perimetry, and fundus photography.
  • Analyzed S-F relationships using the Garway-Heath map and evaluated ONH rotation/tilt effects via linear regression.

Main Results:

  • ONH ovality correlated with nasal pRNFLT in OAG and healthy HM eyes, but not in HMG eyes.
  • ONH rotation associated with inferior nasal pRNFLT in HMG eyes and inferior temporal/nasal pRNFLT in healthy HM eyes.
  • Strongest pRNFLT-VFSL relationships found in the inferior temporal sector for HMG (R=0.567) and superior temporal for OAG (R=0.638).

Conclusions:

  • Optic disc rotation and tilt may alter pRNFL distribution in HM eyes, leading to abnormal S-F correspondence.
  • The inferior temporal sector's S-F relationship analysis is valuable for assessing HMG.
  • These findings highlight the importance of considering ONH morphology in glaucoma evaluation, especially in myopic patients.
