Background:

Netarsudil 0.02% is a Rho kinase inhibitor used for glaucoma treatment.
Corneal verticillata is a known side effect of netarsudil therapy.

Observation:

Bilateral conjunctival pigmentation and cornea verticillata developed in a patient after 21 months of topical netarsudil use.
The conjunctival pigmentation resembled primary acquired melanosis.
Both conjunctival and corneal findings resolved 14 months after discontinuing netarsudil.

This case report identifies conjunctival pigmentation as a potential, previously unreported side effect of topical netarsudil.
