Idiopathic hair casts in children were effectively treated with tretinoin lotion. Microscopic analysis suggests these hair casts originate from the infrainfundibulum.

    Area of Science:

    • Dermatology
    • Pediatric Dermatology
    • Hair and Scalp Disorders

    Background:

    • Hair casts are a poorly understood follicular condition.
    • Idiopathic hair casts present a diagnostic challenge, particularly in pediatric cases.
    • Understanding the origin and effective treatment of hair casts is crucial for pediatric dermatology.

    Observation:

    • Two pediatric patients, aged 5 and 7 years, presented with idiopathic hair casts.
    • A 0.025% tretinoin lotion demonstrated efficacy in resolving the hair casts.
    • Microscopic investigations, including light and transmission electron microscopy, were performed.

    Findings:

    • Histopathological analysis of hair casts revealed components of both the inner and outer root sheath.
    • These findings suggest an infrainfundibular origin for idiopathic hair casts.

    Related Experiment Videos

  • Literature review identified two patient groups: idiopathic (diffuse involvement) and secondary to scalp inflammation.
  • Implications:

    • Tretinoin lotion may be a viable therapeutic option for pediatric idiopathic hair casts.
    • The infrainfundibular origin provides new insight into the pathophysiology of hair casts.
    • Distinguishing between idiopathic and secondary hair casts is important for appropriate management in children.
