Background:

  • Preoperative assessment of maxillary sinus computed tomography (CT) characteristics is vital for surgical integrity.
  • Existing classifications may not encompass all anatomical variations encountered.

Purpose of the Study:

  • To identify and classify previously unpublished variations of the maxillary sinus using CT scans.
  • To enhance surgical planning and communication in otorhinolaryngology.

Main Methods:

  • Analysis of 110 paranasal CT scans (220 sides).
  • Axial, coronal, and sagittal multiplanar reconstructions were utilized.
  • Classification of maxillary sinus floor, lateral wall, and orbital floor variations.

Main Results:

  • Maxillary sinus floor: Type 1 (53.2%), Type 2 (29.1%), Type 3 (10%), Type 4 (7.7%).
  • Lateral maxillary sinus wall: Type 1 (32.7%), Type 2 (65%), Type 3 (2.3%).
  • Orbital floor: Type 1 (0.9%), Type 2 (21.3%), Type 3 (50.5%), Type 4 (27.3%). Significant gender differences noted for floor and lateral wall. Asymmetry observed in 22.7% (floor), 16% (lateral wall), and 30% (orbital floor).

Conclusions:

  • The study introduces novel classifications for maxillary sinus variations based on CT findings.
  • Increased surgeon awareness of these variations can improve surgical outcomes and patient safety.
  • Findings are valuable for training medical residents in endoscopic sinus surgery.
