Background:

Medial meniscal ramp tears involve peripheral detachment of the posterior horn, frequently associated with anterior cruciate ligament (ACL) rupture.
These tears result from detachment at the meniscocapsular junction or meniscotibial ligament, influenced by semimembranosus tendon mechanics.

Discussion:

Existing classification systems for ramp tears lack validation, reliability, and outcome analysis.
Diagnosis is challenging, with MRI showing moderate sensitivity; arthroscopy remains the gold standard.
Risk factors include specific ligament injuries, tibial slope, alignment, bone contusions, and chronic injury duration.
