    Background:

    • While arterial supply remained adequate, venous return through the radial veins can be compromised.
    • The inclusion of an additional, supernumerary vein in the flap is a proposed solution.
    • Suturing this auxiliary vein to a recipient site superficial vein facilitates unimpeded venous drainage.

    Implications:

    • This technique enhances the safety and reliability of osteocutaneous radial artery forearm flaps.
    • Ensuring adequate venous outflow is critical for flap survival and successful reconstruction.
    • The findings suggest a modification to standard surgical practice for improved patient outcomes.
