Background:
- Sepsis survivors are at risk for developing left ventricular systolic dysfunction (LVSD) and subsequent heart failure.
- Guideline-directed medical therapy (GDMT) is standard for non-sepsis-related heart failure but its efficacy in sepsis survivors with LVSD is less understood.
Purpose of the Study:
- To assess the effectiveness of guideline-directed medical therapy (GDMT) in sepsis survivors diagnosed with left ventricular systolic dysfunction (LVSD).
- To evaluate changes in cardiac function and dimensions following GDMT in this patient population.
Main Methods:
- Retrospective analysis of 25 sepsis survivors with suspected myocardial injury or heart failure and diagnosed LVSD at a UK cardiac center.
- Collection and analysis of clinical data and transthoracic echocardiography (TTE) parameters, including LVEF, LVIDd, LVIDs, and TAPSE.
- Assessment of changes in cardiac function and dimensions after a median of 6 months of GDMT in patients with follow-up TTE scans.
Main Results:
- 11 out of 25 sepsis survivors (44%) presented with LVSD (LVEF < 50%).
- Patients with LVSD showed significantly greater LV end-systolic diameter (LVIDs) compared to those without LVSD (p=0.027).
- Nine patients with LVSD received GDMT and underwent repeat TTE; 8/9 demonstrated significant LV systolic functional recovery (>5% LVEF increase) with a mean LVEF improvement of 16 ± 11%.
Conclusions:
- Guideline-directed medical therapy (GDMT) appears beneficial for improving left ventricular systolic function in sepsis survivors with LVSD.
- Further multi-center validation studies are recommended to confirm the therapeutic value of GDMT in post-sepsis heart failure.
