Narcolepsy

Narcolepsy is a chronic sleep disorder characterized by pervasive, uncontrolled sleepiness and other sleep disturbances. One of its hallmark symptoms is an abrupt transition to REM sleep upon falling asleep, which causes symptoms typically associated with this phase to occur unexpectedly during wakefulness. Background:

A 29-year-old woman with ADHD, PTSD, type 2 diabetes, and metabolic conditions presented with excessive daytime sleepiness.
Diagnostic overshadowing by psychiatric conditions delayed the identification of a primary sleep disorder.

Purpose of the Study:

To report a case of narcolepsy type 2 misdiagnosed due to psychiatric comorbidities.
To emphasize the importance of considering sleep disorders in patients with complex medical histories.

Main Methods:

Polysomnography
Multiple sleep latency tests
Cerebrospinal fluid analysis

Main Results:

Narcolepsy type 2 was diagnosed based on objective sleep testing and CSF findings.
Methylphenidate treatment effectively reduced excessive daytime sleepiness and sleep duration.

Conclusions:

Excessive daytime sleepiness can be a primary narcolepsy symptom, even in patients with psychiatric and metabolic disorders.
Prompt diagnosis and treatment of sleep disorders are essential to prevent functional impairment.
