Abstract:
Organized by the nurse coordinator of the territorial professional health community, the homecare support program team and the cardiology department of the Roanne hospital center, the discharge program for heart failure patients aims to strengthen coordination and reduce the rate of re-hospitalization. It lasts from eight weeks to six months, depending on the needs of the patients being cared for. At the same time, a training program has been developed for healthcare professionals involved in patient follow-up.
