Abstract:
Heart failure mainly affects the elderly, who are often frail and suffer from co-morbidities. Frequent rehospitalization, from both cardiac and extra-cardiac causes, impacts on their quality of life. An acute geriatric unit with a cardio-geriatric focus has been developed to provide multidisciplinary, coordinated care. This model combines geriatric and cardiological expertise, with the aim of reducing re-hospitalization, optimizing treatment and promoting an adapted care pathway.
