Background:

Aligning digitalization across health systems presents significant challenges.
Academic health centres (AHCs) often develop digital capacities independently.
Understanding factors influencing internal digital capacity and system-level alignment is crucial.

Purpose of the Study:

To trace digital development in six Ontario AHCs over three decades.
To identify factors influencing internal digital capacities.
To assess the contribution of AHC digitalization to system-level policy objectives, particularly patient-centred integrated care.

Main Methods:

Case study methodology involving six academic health centres in Ontario, Canada.
Data collection through document review and interviews with AHC leaders.
Data collection conducted in 2019 and repeated in 2021 to capture the impact of the COVID-19 pandemic and system reforms.

Main Results:

Digitalization efforts within AHCs showed minimal intersection or coordination with provincial initiatives over three decades.
The COVID-19 pandemic positively influenced the perceived value of digitalization.
Recent policy shifts towards patient-centred integrated care present improved prospects for alignment.
