NETs in the spotlight: exploring NETosis markers for tracking disease activity in IgA vasculitis

Purpose of the Study:

- A longitudinal study was conducted with 33 pediatric IgAV patients and 26 healthy controls.
- Blood and urine samples were collected during active and inactive disease phases.
- NETosis markers including cell-free DNA (cf-DNA), neutrophil elastase (NE), myeloperoxidase (MPO), and citrullinated histone H3 (cit-H3) were quantified using ELISA.
Main Results:
- Serum cf-DNA levels were significantly higher in patients with active IgAV compared to controls and inactive IgAV patients.
- Urine MPO levels were lower in active IgAV patients than controls, while urine cit-H3 levels were elevated in both active and inactive IgAV phases compared to controls.
- A serum cf-DNA threshold of 935 ng/ml demonstrated 93% sensitivity and 72% specificity for identifying active IgAV.
Conclusions:
- Elevated serum cf-DNA and urinary cit-H3 suggest a role for NETosis in IgAV activity.
- These NETosis markers may serve as potential indicators for monitoring disease activity in IgAV.
- Further research is needed to standardize NETosis marker assessment protocols for IgAV.
