Summary

Transcatheter aortic valve replacement (TAVR) may improve outcomes for patients with moderate aortic stenosis (AS). This study suggests TAVR could reduce mortality and heart failure hospitalizations in this patient group.

Background:

  • Moderate aortic stenosis (AS) is associated with reduced long-term survival.
  • The benefit of transcatheter aortic valve replacement (TAVR) in moderate AS remains unclear.
  • Current guidelines typically reserve TAVR for severe AS.

Purpose of the Study:

  • To investigate the clinical outcomes of TAVR in patients with moderate AS.
  • To compare TAVR versus clinical surveillance in moderate AS patients.
  • To assess TAVR's impact on mortality and heart failure hospitalizations.

Main Methods:

  • Univariable and multivariable Cox regression analyses were used.
  • Propensity score-matched comparison between TAVR and clinical surveillance groups.
  • Inclusion of patients with moderate AS irrespective of left ventricular ejection fraction (LVEF).

Main Results:

  • TAVR was associated with significantly lower all-cause mortality (aHR: 0.51).
  • TAVR reduced the composite endpoint of mortality and heart failure hospitalizations (aHR: 0.53).
