Area of Science:

  • Gastroenterology
  • Endoscopic procedures
  • Biliary interventions

Background:

  • Embedded transpapillary self-expandable metal stents (SEMS) pose extraction challenges.
  • Standard extraction methods for refractory embedded SEMS often result in failure.

Purpose of the Study:

  • To evaluate the feasibility and outcomes of a novel balloon dilation-assisted extraction technique for embedded SEMS.
  • To assess the efficacy and safety of this new endoscopic approach.

Main Methods:

  • Retrospective analysis of consecutive patients undergoing endoscopic balloon-assisted stent extraction.
  • Data collection included baseline, procedural, and follow-up information.

Main Results:

  • Successful SEMS extraction was achieved in 83.3% of 12 patients with embedded transpapillary SEMS.
  • The technique was effective even after previous failed extraction attempts and SEMS-in-SEMS placement.
  • Adverse events were generally low-grade (II-III), with no major procedure-related complications.

Conclusions:

  • Endoscopic balloon-assisted stent extraction is a promising method for embedded SEMS.
  • The technique offers high efficacy and safety using standard endoscopic tools.
  • It should be considered for difficult SEMS extractions.
