Background:

  • Microtia reconstruction commonly uses autologous rib cartilage grafts (ARCG) or porous polyethylene (PPE) implants.
  • ARCG is the traditional standard, but PPE offers the advantage of avoiding donor site morbidity.

Purpose of the Study:

  • To compare the outcomes of microtia reconstruction using ARCG versus PPE implants.
  • To systematically analyze infection rates, framework exposure, redo procedures, aesthetic results, and patient satisfaction.

Main Methods:

  • A systematic literature search was conducted for studies published between 2000 and 2023.
  • Meta-analysis of 11 included studies (3816 patients) compared ARCG and PPE outcomes using a random-effects model.

Main Results:

  • Porous polyethylene (PPE) implants showed higher pooled rates for infection (3.18%), framework exposure (6.97%), and redo procedures (4.88%) compared to autologous rib cartilage grafts (ARCG).
  • Aesthetic outcomes and patient satisfaction showed varied results across studies, with no significant differences in quality of life.
  • Significant heterogeneity (I² = 39%-68%) was noted across the included studies.

Conclusions:

  • Porous polyethylene (PPE) implants may be associated with increased risks of infection, framework exposure, and repeat surgeries compared to autologous rib cartilage grafts (ARCG).
  • The quality of existing studies and observed heterogeneity underscore the need for more rigorous research to definitively compare these reconstructive techniques.
