[Proctitis: A Clinical Case of Unusual Etiology]
1Hospital Dr. Gustavo Fricke, Universidad de Valparaíso, Viña del Mar, Chile.
Abstract:
Proctitis frequently appears in inflammatory bowel diseases, mainly in ulcerative colitis. On the other hand, an increased incidence of sexually transmitted infections (STIs) has been described, which can start as proctitis. Additionally, the infectious etiology includes fecal-oral transmission pathogens as proctitis triggers. Similarly, non-infectious causes also play a role in the etiopathogenesis. The case is described of a 34-year-old man whit HIV on antiretroviral treatment, raising questions regarding management and the need to clarify the diagnosis in the context of a condition that was inadequately advancing and ultimately culminated in an unexpected turn of events, revealing that the cause of his ailment was one of the most common sexually transmitted infections (STIs) worldwide, with accessible diagnosis and treatment. The patient was treated with antimicrobials for 3 weeks and showed favorable improvement.
Insights
Proctitis can stem from inflammatory bowel disease or sexually transmitted infections (STIs). A case highlights an HIV-positive man whose proctitis was successfully treated as a common STI.
Area of Science:
- Gastroenterology
- Infectious Diseases
- Public Health
Background:
- Proctitis is a common symptom in inflammatory bowel diseases, particularly ulcerative colitis.
- Sexually transmitted infections (STIs) can also present as proctitis, increasing their incidence.
- Both infectious and non-infectious etiologies contribute to proctitis development.
Purpose of the Study:
- To describe a challenging case of proctitis in an HIV-positive patient on antiretroviral therapy.
- To emphasize the importance of accurate diagnosis in advancing proctitis cases.
- To highlight the successful management of proctitis caused by a common STI.
Main Methods:
- Case report of a 34-year-old male patient with HIV.
- Diagnostic evaluation to determine the cause of refractory proctitis.
- Antimicrobial treatment for a confirmed sexually transmitted infection.
Main Results:
- The patient's proctitis, initially poorly managed, was ultimately diagnosed as a common STI.
- Diagnosis and treatment of the STI led to favorable clinical improvement.
- The case underscores the treatability of STIs presenting as proctitis.
Conclusions:
- Accurate diagnosis is crucial for effective proctitis management, especially in immunocompromised individuals.
- Sexually transmitted infections are a significant and treatable cause of proctitis.
- Prompt antimicrobial therapy for STIs results in positive patient outcomes.

Inflammatory Bowel Disease I: Ulcerative Colitis
Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...

Inflammatory Bowel Disease II: Crohn's Disease
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.
Crohn's disease
Crohn's disease is a chronic, systemic inflammatory bowel disease (IBD) that predominantly affects the gastrointestinal tract. It is marked by...
Peptic Ulcer Disease III: Clinical Manifestations and Diagnostic Studies
Few clinical manifestations differentiate gastric ulcers from duodenal ulcers. Distinctions in the location, timing, and pain relief are crucial for healthcare providers in differentiating between gastric and duodenal ulcers during clinical assessments.
