MicroRNA (miRNA) are short, regulatory RNA transcribed from introns (non-coding regions of a gene) or intergenic regions (stretches of DNA present between genes). Several processing steps are required to form biologically active, mature miRNA. The initial transcript, called primary miRNA (pri-mRNA), base-pairs with itself, forming a stem-loop structure. Summary

Overexpression of miR-146b and miR-221 in papillary thyroid carcinoma (PTC) tissues may indicate lymph node metastasis (LNM) and a poorer prognosis. Larger tumors in metastatic PTC correlate with these miRNA changes.

Background:

  • Papillary thyroid carcinoma (PTC) is the most common endocrine malignancy.
  • Lymph node metastasis (LNM) is a critical factor influencing PTC prognosis.
  • MicroRNAs (miRNAs) play significant roles in cancer development and progression.

Purpose of the Study:

  • To investigate the prognostic value of specific miRNAs in PTC.
  • To assess the association between miRNA expression profiles and LNM in PTC.
  • To develop predictive models for LNM using clinicopathological features and miRNA expression.

Main Methods:

  • Retrospective analysis of 47 lymph node-metastatic and 46 non-metastatic PTC patients.
  • Quantitative reverse transcription-polymerase chain reaction (RT-PCR) to analyze miR-21, miR-146b, miR-221, and miR-222 expression.
  • Correlation analysis between miRNA expression, clinicopathological features, and LNM status.

Main Results:

  • miR-146b and miR-221 were significantly overexpressed in metastatic PTC tissues compared to non-metastatic tissues (p < 0.001 and p = 0.009, respectively).
  • miRNA expression profiles in metastatic lymph nodes mirrored those in primary tumors.
  • Overexpression of miR-146b and miR-221 in tumor tissue was associated with larger tumor size (>1 cm) in metastatic PTC (p = 0.036).
  • miR-222 was overexpressed in metastatic lymph nodes in the presence of larger tumors (p = 0.035).
  • miR-146b upregulation was observed in the absence of peritumoral lymphocyte infiltration (p = 0.006).

Conclusions:

  • Overexpression of miR-146b and miR-221 in PTC tissues is a potential indicator of LNM and poorer prognosis.
  • Clinicopathological factors like tumor size and intratumoral fibrosis may also contribute to predicting PTC prognosis.
