Bilateral Periprosthetic Cysts After Ceramic-on-Polyethylene Direct Anterior Approach Total Hip Arthroplasty: A Case

Case:
A 59-year-old man presented 18 months following bilateral total hip arthroplasty (THA) with ceramic-on-polyethylene bearings through the direct anterior approach, with progressive bilateral groin pain and swellings around his upper thighs since 2 months. He was advised surgery considering the possibility of a periprosthetic joint infection. Intraoperatively, large pedunculated periprosthetic synovial cysts were noted bilaterally. They were excised en masse, and the resulting defects were repaired. At 15 months following surgery, he had no recurrence of symptoms.
Conclusion:
The possibility of periprosthetic cystic lesions must be considered in the differential diagnosis of inexplicable pain following THA.
