Nonoperating room anesthesia services present significant challenges for anesthesiologists due to the unfamiliar environment, remote location, and the potentially limited access to essential equipment and anesthesia-trained personnel. This case report details an incident of ventilation failure with a Bain circuit during electroconvulsive therapy, attributed to a leak in the fresh gas flows. We promptly recognized the issue and utilized a bag-valve-mask to ventilate the patient. This event highlights the indispensable role of a readily available bag-valve-mask to ensure timely and effective management of any ventilation compromise, particularly in resource-limited settings such as nonoperating room locations.
