Posterior mediastinal hydatid cysts are rare but can compress vital structures. Multimodality imaging accurately diagnoses these cysts, guiding crucial management to prevent complications.

Background:
- Posterior mediastinal masses are uncommon and often asymptomatic until significant compression of adjacent structures occurs.
- Computed tomography (CT) and cardiac magnetic resonance (CMR) are essential for precise localization and characterization of mediastinal lesions.
Observation:
- A cystic mass in the posterior mediastinum was identified in a young woman, extending into the interatrial septum and left atrium (LA).
- Echocardiography, cardiac CT, and CMR confirmed the mass's location and its protrusion into cardiac chambers.
Findings:
- Diagnostic imaging and subsequent histopathologic examination confirmed the mass as a hydatid cyst.
- The cyst's location mimicked a cardiac mass on initial echocardiographic evaluation.
Implications:
- Mediastinal hydatid cysts, though rare, can lead to severe complications like organ compression or rupture if not diagnosed and managed promptly.
