Background:

Complex sacral fractures require surgical stabilization.
Postoperative complications can arise from spinal procedures.

Observation:

A 73-year-old patient developed a complex sacral fracture and underwent surgery.
Postoperative brain computed tomography (CT) revealed fat droplets within the intracranial subarachnoid space and lateral ventricles.

Findings:

The observed intracranial fat droplets originated from the sacral thecal sac.
A tear in the sacral thecal sac allowed fat to migrate into the cerebrospinal fluid.

Implications:

Intracranial fat embolism is a potential, albeit rare, complication following sacral surgery.
Radiologists and surgeons should be aware of this phenomenon for accurate diagnosis and patient management.
