  • A 35-year-old female with situs inversus totalis presented for a third kidney transplant after two prior graft failures.
  • The first graft was a right-sided living related transplant, failing after 3 years.
  • The second graft, a left-sided related transplant, thrombosed and was removed within the first week.

Findings:

  • The patient successfully received a third related kidney transplant on the left side.
  • The surgical team managed the altered vascular anatomy associated with situs inversus totalis.
  • Post-operative outcomes for retransplantation in this patient were favorable.

Implications:

  • This case demonstrates that retransplantation in situs inversus totalis patients can be successful.
  • Careful consideration of aberrant vascular anatomy is crucial for surgical planning.
  • Further research into transplant outcomes in rare anatomical variations is warranted.
