Area of Science:

  • Radiology
  • Orthopedics
  • Medical Imaging

Background:

  • Osteoarthritis (OA) is a progressive joint disease leading to disability.
  • Radiological assessments are crucial for detecting subtle OA changes.
  • Early diagnosis of knee OA necessitates improved radiographic assessment methods.

Purpose of the Study:

  • To evaluate six radiographic measurement methods for diagnosing knee osteoarthritis.
  • To assess the inter-observer reliability of these radiographic measurements.
  • To compare measurements between osteoarthritic and non-osteoarthritic groups.

Main Methods:

  • Retrospective cross-sectional imaging study of knee X-rays (January 2022 - December 2023).
  • Two blinded radiologists reviewed images, classifying subjects using the Kellgren-Lawrence system.
  • Six radiographic measurements, including femorotibial angle and medial joint space width, were evaluated.

Main Results:

  • 353 subjects included; 67% classified as normal, 33% showed OA signs.
  • Medial joint space width was significantly lower in the osteoarthritic group (P < 0.001).
  • Increased distance between medial tibial eminence and medial femoral condyle correlated with higher OA likelihood (OR: 1.2).

Conclusions:

  • Evaluated radiographic measurements show potential as diagnostic tools for knee OA.
  • These methods can identify various stages of knee osteoarthritis severity.
  • Radiographic measurements enhance the diagnostic performance of the Kellgren-Lawrence classification system for knee OA.
