Insights
Carotid atherosclerosis, a cause of stroke, is increasingly assessed by plaque features beyond stenosis severity. These plaque characteristics guide treatment decisions for preventing future ischemic events.
Area of Science:
- Vascular medicine
- Neurology
- Radiology
Background:
- Carotid atherosclerosis is a significant cause of embolic stroke and ischemic events.
- Stenosis severity is a primary factor in treatment decisions (medical vs. interventional therapy).
- Emerging plaque characteristics are gaining importance in risk stratification.
Purpose of the Study:
- To review current carotid plaque analysis techniques.
- To discuss the impact of plaque analysis on clinical practice and guidelines.
- To highlight specific plaque features associated with stroke risk.
Main Methods:
- Review of current literature on carotid plaque imaging modalities.
- Analysis of plaque features such as total plaque area, juxtaluminal black area, intraplaque hemorrhage, and plaque echolucency.
- Discussion of how these features influence therapeutic choices.
Main Results:
- Specific plaque features (e.g., intraplaque hemorrhage, echolucent plaques) are linked to a higher risk of stroke.
- These features are increasingly influencing treatment decisions beyond traditional stenosis severity.
- Advanced imaging allows for detailed characterization of atherosclerotic plaques.
Conclusions:
- Carotid plaque analysis, incorporating specific features, is crucial for accurate stroke risk assessment.
- These detailed analyses impact current guidelines and clinical management strategies.
- Further research into plaque imaging may refine therapeutic interventions for carotid atherosclerosis.
Abstract:
Carotid atherosclerosis is an important yet declining etiology of embolic stroke and cerebrovascular or retinal ischemic events with an embolic source. The severity of stenosis is the most important anatomic factor for deciding whether to use optimized medical therapy alone or interventional therapy. Recently, specific features of the atherosclerotic plaque have attracted increasing attention; for example, total plaque area, juxtaluminal black area, intraplaque hemorrhage, and mainly echolucent plaques are associated with high risk of late stroke, and these features now influence the therapeutic choices. This review describes the current state of carotid plaque analysis using different image modalities and its effect on current guidelines and clinical practice.
