Collecting duct carcinoma (CDC), also known as Bellini duct carcinoma, is a highly aggressive and extremely rare subtype of renal cell carcinoma. Previous studies have suggested that CDC generally exhibits low CXCR4 expression, leading to negative or weak 68 Ga-pentixafor uptake. However, we report a case of 68 Ga-Pentixafor uptake in CDC on PET/CT. This finding challenges the conventional understanding of chemokine characteristics in CDC and underscores the importance of tumor heterogeneity, potentially providing new insights for precision treatment strategies.
