Abstract:
Social connection is essential to the health and well-being of older adults, particularly those living with chronic conditions and social or environmental barriers. This article highlights public health strategies and community-based resources that promote and sustain social connections among at-risk older adults. Emphasis is placed on cross-sector approaches, including public awareness campaigns and person-centered social prescriptions. A key focus is the integration of aging care services with health care delivery systems to support socially at-risk older adults. Examples of successful partnerships between community-based organizations and health care systems aimed at improving social connectedness in older adult populations are presented.
