Background:

  • Erythrodermic psoriasis (EP) is a rare, severe inflammatory skin disease affecting over 90% of body surface area (BSA), often resistant to conventional treatments.
  • The interleukin-17 (IL-17) pathway is implicated in EP pathogenesis, though less understood than plaque psoriasis.
  • Brodalumab, an IL-17 receptor A antagonist, offers broader IL-17 pathway blockade compared to other biologics.

Purpose of the Study:

  • To report a case of successful treatment of erythrodermic psoriasis (EP) with brodalumab.
  • To evaluate the efficacy and safety of brodalumab in a patient with severe, treatment-naïve EP and comorbidities.

Main Methods:

  • A 57-year-old woman with newly diagnosed plaque psoriasis rapidly progressing to EP was treated with brodalumab after prophylactic treatment for latent tuberculosis.
  • Clinical assessment included body surface area (BSA) involvement, Psoriasis Area and Severity Index (PASI), and Dermatology Life Quality Index (DLQI).
  • Patient received brodalumab at the labeled dosage, with outcomes assessed at two and four weeks.

Main Results:

  • Rapid and dramatic improvement was observed within two weeks, with significant reduction in erythema, scaling, and associated symptoms (itching, burning).
  • Ocular involvement, including conjunctival redness, completely resolved.
  • After four weeks, PASI score decreased to 2, BSA involvement reduced to 5%, with substantial improvement in quality of life and no reported adverse events.

Conclusions:

  • Brodalumab demonstrated rapid clinical efficacy and excellent safety in treating a patient with severe erythrodermic psoriasis.
  • The drug's rapid action may help avoid systemic steroid therapy and associated adverse events, reducing hospitalization.
  • Brodalumab represents a potential therapeutic option for EP, offering significant clinical benefit and improved patient management.
