Regional Anesthesia in Brugada Syndrome-Worth the Risk? A Dual-Case Report

Abstract:
Because of a greater risk of ventricular fibrillation and subsequent cardiac arrest, patients with Brugada syndrome are especially challenging for anesthesiologists. Local anesthetics must be used cautiously in these individuals as they block the same sodium channels that are intrinsically dysfunctional in Brugada syndrome. We present the anesthetic management of a patient with Brugada syndrome who underwent consecutive total shoulder arthroplasties under general anesthesia-the first with a nerve block catheter and the second with a single-shot nerve block. Her surgical plans necessitated a multidisciplinary approach involving anesthesiology, intensive care, acute pain service, cardiology, orthopedic surgery, and cardiothoracic surgery.
