Immunosuppressed patients on methotrexate therapy face risks of opportunistic infections. A rare case involved concurrent Epstein-Barr virus-associated mucocutaneous ulcers and Purpureocillium lilacinum causing eyelid ulceration.

- Discontinuation of MTX and treatment with oral voriconazole led to the resolution of the ulcer.
