Altitude-Associated Central Apnea in Continuous Positive Airway Pressure Therapy

High altitude can trigger central sleep apnea in patients with obstructive sleep apnea, even with CPAP therapy. Supplemental oxygen can resolve these altitude-associated central apneas.

Background:

Observation:
- A 64-year-old male on CPAP for severe OSA experienced central apneas exclusively at high altitudes (2,600m).
- Symptoms and central apneas occurred only during sleep at high altitudes.
- Other causes of central apnea were excluded.
Findings:
- Altitude-associated central apnea is a recognized condition above 1,600m.
- Supplemental oxygen effectively corrected the central apneas in this case.
- Acetazolamide is another recommended treatment.
Implications:
- Clinicians should consider altitude changes in patients with OSA on positive airway pressure (PAP) therapy.
- Occupational and recreational travel to high altitudes can precipitate sleep-disordered breathing.
- Awareness is crucial for managing OSA patients in diverse environmental conditions.
