Summary

Rituximab (anti-CD20 antibody) showed limited efficacy in most neurosarcoidosis patients, with 73.7% experiencing treatment failure. However, a small subset with multiple cranial neuropathies may still benefit, even after other treatments failed.

Background:

  • Neurosarcoidosis is a rare inflammatory disease affecting the central nervous system.
  • Rituximab, a B-cell depleting therapy, has been investigated for neurosarcoidosis, but clinical evidence remains scarce.

Purpose of the Study:

  • To evaluate the efficacy and safety of rituximab in patients diagnosed with neurosarcoidosis.
  • To identify patient subgroups that may respond to rituximab treatment.

Main Methods:

  • Retrospective study of 19 neurosarcoidosis patients treated with rituximab across four US centers.
  • Treatment failure was defined by persistent/worsening MRI inflammatory burden or inability to taper prednisone.

Main Results:

  • Rituximab failed in 73.7% of patients, with relapse or progression in 85.7% and steroid-sparing failure in 14.3%.
  • The median time to first relapse was 7 months.
  • Five patients (26.3%) responded, predominantly those with multiple cranial neuropathies treated with rituximab as third-line or later therapy.

Conclusions:

  • Rituximab demonstrates limited overall effectiveness in neurosarcoidosis management.
  • A specific subgroup, particularly those with multiple cranial neuropathies, may experience benefits from rituximab, even in later lines of therapy.
