Suprachoroidal Triamcinolone Acetonide for the Management of Retinal Vasculitis

Purpose:
To evaluate changes in vascular leakage on fluorescein angiogram (FA) after suprachoroidal triamcinolone acetonide (SCS-TA) in patients with retinal vasculitis.
Methods:
In this single-center, retrospective case series, the medical records of patients receiving a single SCS-TA injection for retinal vasculitis between January and December 2024 were reviewed. The primary outcome was the change in FA grade after injection using the Angiography Scoring for Uveitis Working Group scoring system. Secondary outcomes included best-corrected visual acuity (BCVA) and intraocular pressure (IOP) at baseline and last follow-up.
Results:
Ten eyes of eight patients underwent SCS-TA injection with a mean follow-up time of 115.1 days (SD = 62.4). Mean FA grade improved from 10.8 to 6.0 (mean difference 4.8, p = 0.004). Eight of the 10 eyes demonstrated an improvement in FA grade, one eye could not be assessed due to poor-quality FA at follow-up. There was no change in BCVA in six eyes, while BCVA improved in two eyes and worsened in two eyes. No eyes experienced a BCVA change of three lines or more. A clinically significant IOP elevation was observed in three of ten eyes, all of which had a history of prior steroid response.
Conclusion:
SCS-TA resulted in significant improvement of retinal vasculitis as assessed by FA grade, with a manageable side effect profile. However, caution should be maintained for patients with a history of steroid-induced glaucoma.s.
