Background:

  • Gait dysfunction is a primary challenge in Parkinson disease (PD) management.
  • Reduced vestibular efficacy is implicated in PD-related imbalance.
  • Targeting axial muscles via upper limb exercise may improve PD symptoms.

Purpose of the Study:

  • To investigate if upper limb aerobic exercise can enhance postural control and motor function in PD patients.
  • To compare the effects of supervised exercise (SE) versus voluntary exercise (VE).

Main Methods:

  • Twenty-eight PD patients underwent 2 months of exercise.
  • Evaluations included dynamic posturography (Sensory Integration Test - SOT) and clinical measures (UPDRS Part III, Hoehn and Yahr scale).
  • Participants were divided into SE and VE groups.

Main Results:

  • Supervised exercise (SE) significantly improved vestibular SOT scores (p=0.017).
  • No significant changes in somatosensory or visual SOT scores were observed.
  • SE group showed significant improvements in Hoehn and Yahr scale (p=0.031) and UPDRS Part III (p=0.007).

Conclusions:

  • Active assistive SE enhances vestibular utilization and motor performance in Parkinson disease.
  • This exercise modality is vital for PD patients, particularly those with lower limb impairments.
