Pathological laughter is a rare, temporary side effect of atomoxetine (a selective norepinephrine reuptake inhibitor) in children with autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD). Monitoring mood-related side effects is crucial for personalized treatment.

- No recurrence of pathological laughter was observed after atomoxetine dosage was increased.

- Clinicians should monitor for rare mood-related side effects like pathological laughter in children with neurodevelopmental disorders on atomoxetine.
- The case highlights the importance of considering catecholamine influence on mood regulation in this population.
- Further research is warranted to understand the mechanisms and risk factors for atomoxetine-induced pathological laughter.
