Summary

Systemic arterial stiffness is linked to glaucoma. Acceleration plethysmography (APG) parameters, particularly b and d-waves, showed differences between glaucoma patients and controls, suggesting APG may reflect vascular changes in glaucoma.

Background:

  • Systemic arterial stiffness and atherosclerosis are increasingly implicated in glaucoma pathogenesis.
  • Previous research linked glaucoma to vascular stiffness markers, but the role of acceleration plethysmography (APG) remained unexplored.

Purpose of the Study:

  • To investigate the association between APG parameters and the presence of glaucoma.
  • To explore potential vascular alterations reflected by APG in glaucoma patients.

Main Methods:

  • A study involving 701 open-angle glaucoma patients (POAG/EXG) and 94 controls.
  • APG was measured using a sphygmograph, analyzing waveform amplitudes and derived vascular types.
  • Statistical comparisons were made between glaucoma cases and controls.

Main Results:

  • Significant differences in b-waves (p=0.050) and d-waves (p=0.014) of accelerated pulse waves were observed between glaucoma patients and controls.
  • Exfoliation glaucoma (EXG) patients showed a lower frequency of Type A and higher frequency of Type C vascular patterns compared to controls and POAG.
  • Multivariate analysis identified age, sex, diastolic blood pressure, and pulse rate as significant factors associated with vascular type.

Conclusions:

  • This is the first large-scale study examining the relationship between APG and glaucoma.
  • APG parameters may serve as indicators of systemic vascular alterations in glaucoma patients, especially in the elderly and those with EXG.
