Minimally invasive approach for the surgical treatment of Rhinophyma: the effectiveness of Microdebrider

Background:
Rhinophymas are disfiguring disorders of the face that represent the end-stage of sebaceous gland hyperplasia in patients with rosacea. The literature reports different surgical approaches to treat this pathology, but the Microdebrider have proved to be successful in sinonasal surgery as it enables precise control of tissue that needs to be removed, proving to be a safe approach, with good aesthetic results.
Objective:
The aim of this paper is to conduct a pilot study on 10 patients with rhinophyma treated with the use of microdebrider, comparing our results with the most recent articles in literature, carrying out a narrative review.
Methods:
10 cases of rhinophyma were presented. All patients underwent mininvasive surgical treatment based on the use of the Microdebrider and different long-term medications with Fitostimoline gauzes or Condress in the postoperative management. All patients underwent periodic outpatient follow-ups, with a minimum follow-up of 12 months. The aesthetic results were compared with the use of the Vancouver scar scale.
Results:
In all cases, the cosmetic outcomes, at 1 year follow-up, were satisfactory, regardless of the postoperative used medication. The mean of the total VSS values for patients treated with the combination of Fitostimoline gauze and gentamicin was 2.29±1.03SD, while for patients treated with Condress gauze the mean was 2.33±0.47SD. The literature review also confirms these results.
Conclusions:
Microdebrider is a valuable tool for precise and effective surgical treatment of rhinophyma, ensuring good aesthetic results, regardless of the type of dressing used in post-operative management.
