Radial Approach Renal Denervation: A First-in-Man Case With the 4-F TIVUS Catheter

Background:
Renal denervation (RDN) is an effective procedure for the treatment of uncontrolled hypertension, typically performed via the femoral approach. Although RDN is generally safe, vascular complications remain the most frequent adverse events.
Case Summary:
A 57-year-old man with resistant hypertension was enrolled in the Renal Denervation Using Ultrasonic Catheter EmitteD Energy Study (REDUCED 1) clinical study. He underwent bilateral RDN using a novel 4-F ultrasound-based catheter via left radial arterial access. The procedure was uneventful, and at 3-month follow-up, ambulatory and office blood pressure showed marked reductions.
Discussion:
This first-in-human case supports the feasibility and safety of transradial RDN using a dedicated 4-F ultrasound catheter. The radial approach may reduce access-site complications and enable same-day discharge. Future studies are needed to confirm the benefits of this system.
Take-Home Message:
Transradial RDN using a novel 4-F ultrasound catheter is safe, feasible, and may offer procedural and patient-care advantages over femoral access.
