Blinding

Blinding is a commonly used method of not telling participants which treatment a subject is receiving. Blinding is a critical part of a randomized control trial or RCT. It reduces the bias that affects the results. In an RCT, blinding is used in the form of a placebo. A placebo effect occurs when untreated subjects falsely believe they have received the treatment and report improved symptoms. A placebo or a dummy treatment is administered to subjects to negate the bias caused by such an effect.

Background:

  • Lichen simplex chronicus (LSC) is a prevalent itchy skin condition.
  • Topical corticosteroids are the standard first-line treatment.
  • Fractional CO2 laser shows promise for pruritus reduction in various dermatoses.

Purpose of the Study:

  • To compare the efficacy of fractional CO2 laser alone, topical corticosteroids alone, and combination therapy for LSC.
  • To evaluate treatment outcomes using patient and physician-reported measures.

Main Methods:

  • Sixty LSC patients were randomized into three groups: fractional CO2 laser, topical corticosteroid, or combination therapy.
  • Treatments were administered over three months.
  • Disease severity assessed via pruritus scales (VAS) and physician assessments (erythema, lichenification, scaling, excoriation, IGA).

Main Results:

  • Combination therapy achieved the highest rates of IGA score 0/1 (93.3%) and VAS 50% reduction (100%).
  • Fractional CO2 laser monotherapy showed significant improvement (66.7% VAS 50% reduction).
  • Topical corticosteroids alone demonstrated the lowest efficacy rates.

Conclusions:

  • Fractional CO2 laser is an effective treatment modality for LSC.
  • Combining fractional CO2 laser with topical corticosteroids enhances treatment outcomes for LSC.
