The global aging trend has heightened the demand for innovative healthcare solutions. This study introduces a digital healthcare platform addressing key challenges such as information silos, inadequate interdisciplinary collaboration, and care discontinuity. The platform integrates real-time data monitoring, proactive health data collection mechanisms, and intelligent tiered notifications to enhance communication among patients, families, and specialists. It demonstrates adaptability across various clinical scenarios, including long-term care, Hospital-at-Home (HaH), and dementia care. Results reveal significant improvements: case handling time was reduced by 73.8%, follow-up visit durations were cut by 60%, and documentation accuracy was enhanced, aligning with the AMIA 25x5 initiative to minimize documentation burdens. Future expansions include integrating generative AI for clinical decision support and extending applications to acute care, rehabilitation, and remote healthcare.
