  • Haemophilus influenzae is a rare but challenging cause of PJI, often missed by conventional methods due to its fastidious nature.

Observation:

  • A 52-year-old male with rheumatoid arthritis on immunosuppressants developed PJI after hip arthroplasty.
  • Standard cultures were negative, but syndromic multiplex PCR (BioFire® JI panel) rapidly detected H. influenzae.
  • The patient was treated with debridement, antibiotic, and implant retention (DAIR), with targeted ceftriaxone therapy based on PCR results.

Findings:

  • Syndromic multiplex PCR provided rapid (2-hour) identification of H. influenzae.
  • Early pathogen detection enabled timely and targeted antibiotic therapy, de-escalating from broad-spectrum agents.
  • The patient experienced clinical recovery, with symptom resolution and implant retention.

Implications:

  • Rapid molecular diagnostics are crucial for identifying rare pathogens like H. influenzae in PJIs.
  • Timely diagnosis and targeted therapy improve clinical outcomes, reduce hospital stays, and preserve prosthetic implants.
  • This approach enhances antimicrobial stewardship and patient management in complex orthopedic infections.
