Orthognathic Surgery for Temporomandibular Joint Pathology-Induced Facial Asymmetry: A Therapeutic Approach

Abstract:
This case report describes the successful orthodontic and surgical treatment of an 18-year-old female patient with severe craniofacial deformities, such as mandibular retrognathism and facial asymmetry due to unilateral temporomandibular joint (TMJ) pathology. Following the completion of preoperative orthodontic preparation, the patient underwent Le Fort I osteotomy for maxillary impaction and sagittal split ramus osteotomy for mandibular advancement and asymmetry correction. After surgical intervention, final occlusal adjustments were made orthodontically, and the treatment was terminated. At the end of the total active treatment period of ~20 months, it was clinically confirmed that the patient's facial aesthetics improved significantly and a stable, functional occlusion was achieved. It was observed that these successful aesthetic and functional results were maintained during the 3-year follow-up period.
