    Summary

    This study found that most patients with macular disease do not use their best-seeing retinal areas. A new test revealed significantly better visual acuity in many patients, suggesting potential for improved fixation strategies.

    Background:

    • Macular disease significantly impairs central vision, often leading to reduced visual acuity.
    • Conventional visual acuity tests may not identify the best available retinal areas for fixation in patients with macular disease.

    Purpose of the Study:

    • To evaluate a novel "full field E" card test for determining best eccentric visual acuity.
    • To assess if patients with macular disease spontaneously utilize their optimal retinal areas for fixation.

    Main Methods:

    • A series of cards with two-dimensional arrays of Snellen "E"s were used.
    • The "full field E" card presents the same letter to both foveal and parafoveal areas simultaneously.
    • Tested on 37 eyes with Snellen visual acuity of 20/70 or worse.
